3 GEN RSS (Rapid Safe Shaping) Introductory Package... Welcome to a revolutionary new concept in endodontics. With the revolutionary new instruments…the 3GEN Reamers, canal spaces can be shaped in a way never dreamed possible before. In fact, if you are not breaking the instruments, you simply are “not using them properly”! What a paradigm shift that is from instruments we have been forced to use at “snail speed”, (175 – 350 rpm’s), while worrying about what would happen to over stressed instruments. Traditional files break near the tip and are non retrievable. Indeed, although 3GEN Reamers may look similar to traditional endodontic instruments; when it comes to performance it is like comparing a soapbox derby car to a NASCAR finely tuned racecar. Due to the high strength, hybrid cutting flute design and annular groove safety feature; 3GEN Reamers are meant to be used aggressively, at high rpm’s, and repeatedly.

Here is the current recommended technique:
A. After establishing the appropriate access opening and isolation, irrigate the pulp chamber thoroughly with Peridex.

B. With a 25mm, #15 hand file establish the working length with a Root ZX apex locator if possible. Using the same hand file, do preliminary hand filing on the way out of the canal space, before measuring.

C. Using a #20 hand file to the established working length, further enlarge the canal space.

D. At 500 rpm’s use the blue, size 30/.09 taper orifice openers aggressively to begin creating a funnel shape. At this time paper points, gutta percha master cones, and irrigating syringe tips will drop into the canal space.

E. Flush out debris with copious amounts of sterile saline or distilled water. Follow up immediately with an irrigating syringe of full strength Clorox for either control of inflamed pulp or antimicrobial action.

F. Use the appropriate sized 3GEN Reamer, either alone or in combination with hand files to rapidly and safely finish shaping the canal space. In the event a 3GEN Reamer separates at the annular groove location, simply have the dental assistant pick up needle holders and remove the portion in the canal space while you replace with another 3GEN Reamer of the same size. I recommend using the red size 25 3GEN Reamer in small canals, the blue 30 in medium canals, the green 35 in large canals and the black 40 in very large canals. It is not necessary to irrigate between each instrument used, but after any significant canal shaping, I recommend flushing with saline followed with Clorox.

G. I recommend after final instrument shaping, copious flushing with saline, Clorox, and lastly EDTA.

H. Dry with paper points then fill with your normal technique, such as sealer and gutta percha.

I. After cleaning out excess gutta percha, place cotton pellet and TERM (Temporary Endodontic Restorative Material). After light curing the TERM, adjust the bite and take the post op PAX.

Previously if you have used NiTi rotary instruments; use of a 3GEN Reamer aggressively at thousands of rpm’s will seem radically different. As you gain confidence and familiarity with their use it will become easier to shift into the new endodontic reality of one visit (one hour max) root canal treatments. This will be a win-win situation for both patients and the treating dental practice.
