Welcome to a revolutionary new concept in endodontics. With the revolutionary new instruments…the Xtreme Reamers, canal spaces can be shaped in a way never dreamed possible before. In fact, if you are not breaking the instruments, you simply are “not using them properly”! What a paradigm shift that is from instruments we have been forced to use at “snail speed”, (175 – 350 rpm’s), while worrying about what would happen to over stressed instruments. Traditional files break near the tip and are non retrievable. Indeed, although Xtreme Reamers may look similar to traditional endodontic instruments; when it comes to performance it is like comparing a soapbox derby car to a NASCAR finely tuned racecar. Due to the high strength, hybrid cutting flute design and annular groove safety feature; Xtreme Reamers are meant to be used aggressively, at high rpm’s, and repeatedly.

"Extreme Yet Safe"

Xtreme Reamers RSS (rapid safe shaping) Technique:
A. After establishing the appropriate access opening and isolation with an INSTI-DAM, irrigate the pulp chamber thoroughly with a chlorhexidine solution.

B. With a 25mm hand file or 21mm, if short files can be used, establish the working length with an apex locator. Use a size 20 K-Flex hand file in large canals or size 15 in small canals. The closer the hand file comes to matching the size of the apical constriction; the more accurate your apex locator will become. Do preliminary hand filing on the way out of the canal space before measuring.

C. Continue using a #20 K-Flex hand file to the established working length until no additional circumferential filing would be beneficial.

D. At 500 rpm’s use the appropriate 21mm length Xtreme Reamer (S/M/L) to begin creating a funnel shape. At this time irrigating syringe tips will drop into the canal space.

E. Flush out debris with copious amounts of sterile saline or distilled water. Follow up with an irrigating syringe of full strength Clorox for bleeding control of inflamed pulps or antimicrobial action of necrotic pulp tissue.

F. I recommend in small canals use of K-Flex hand files to size 30. In medium and large canals I continue with the K-Flex hand files to size 40. In small canals, I finish the shaping 1 mm short of the working length with a small or medium Xtreme Reamer. In large canals, I again finish the shaping 1mm short of the working length with a large Xtreme Reamer. This provides the safety feature of a second apical stop.

G. It is not necessary to irrigate between each instrument used, but after any significant canal shaping. I recommend flushing out debris with saline followed with Clorox. In the event an Xtreme Reamer separates at the annular groove location, simply pick up the needle holders and remove the separated portion from the canal space.

H. After the final instrument shaping, I recommend copious flushing with EDTA. Dry with paper points then fill with your normal technique, such as sealer and gutta percha.

I. Finish by cleaning out excess gutta percha, If the restorative is not being done immediately, place cotton pellet(s) and TERM (Temporary Endodontic Restorative Material). After light curing the TERM, adjust the bite and take the post op PAX.

J. If you have used NiTi rotary files; use of an Extreme Reamer aggressively at 500 rpm's will seem radically different. As you gain confidence and familiarity with their use, it becomes possible to join the new "one visit endodontics" reality. This will be a win-win situation for both patients and the treating dental practice.
